last compulsory injection!

10th Mar 2009, I had my last compulsory injection; the next compulsory injection will be when I am age 7, hooray!!!
After the injection daddy mummy brought me to Burger King, the uncle in Burger King gave me a Hat!! “I am the KING!!!!”
This is my first time to try out French fried, yummy!!!

Collection of Renunciation Certificate at Malaysia High Comm

Once renunciation of your Malaysian citizenship has been approved, you will receive a letter (email) from Malaysia High Commission asking you to go down to collect your Form K and birth certificate within one month from the date of the e-mail. You have to bring down the following documentation: i)        Singapore Identity Card (photocopy front and back of identity card); ii)      Singapore Citizenship Certificate (photocopy front and back of certificate);  iii)     Singapore Passport (photocopy of page with photograph of passport holder); and iv)       Deed Poll / Baptism (if name has been changed), if applicable. Documents required (Application for adult) 1. Form IM.42 to be duly completed. Obtainable at Forms Counter or Immigration Department website: http://www.imi.gov.my/ a. *Please print on both side of an A4 paper b. *Printing on two pieces of paper will not be accepted 2. 2 recent coloured passport-sized photograph - 3.5cm. x 5cm. with Blue background.
